If you're totally new to survey creations, try starting with template questions we provide.
Click on [Template Questions] button on the top of Edit page, and choose suitable questions in the pop-up window.

You can browse through different categories to find questions you need. The note in the brackets indicates its type. Click on the [Add] button on the right of the question to add it into your survey.
Don't forget to make proper modifications so it fits your own needs.
